Easy to do on Turf War, where there are enough shotguns + ammo to clear out the first floor. Timing can be hard to synch up. When I got it it took my team 3 tries.

We did EXACTLY this and it still didn't trigger. What gives?

Did you make sure to walk away from the piano together afterwards? Apparently this is also a requirement. This is what AndyN said in a previous post about this achievement:

"All 3 players need to start within 1 second of eachother, finish the entire song (which is about a minute) and then leave the piano together."

And there's three different parts, corresponding to the different areas of the piano: a low, med, and high part that must be played. I guess if two people are both playing the medium part, i.e. are close together/on top of each other so that they're playing the same region of the piano, it won't work? I have yet to get this achievement as well yet...none of my friends have the patience to sit at the piano for one minute lol.

I suggest doing the Locksmith Casino level for this achievement. The Piano is located on the top floor - and so is a C4 case. At the beginning of the match, just get one person to be the Pickpocket and give them all of the coins; then, let them get the C4 at the top left corner of the top floor and blow up all of the guards. When blown up by C4, dead NPCs don't leave skeletons behind, thus not allowing any threat to be revived while the piano is being played.

While playing though, make sure each of the three participant is on a different part of the piano (the top part, the middle and the lower; the simple way to check is to see if the players are on top of each other or not; if they are then space out a little bit more). All must start within 1 second of each other as SpartanTwo suggested, and for the whole 1 minute, everyone must keep the move button pressed so that they're constantly moving against the keys/using the piano.
